Hospitals regularly care for people dealing with substance use, withdrawal, or related health crises. Stabilization in an acute care setting addresses the immediate medical concern, but it is often only the beginning of what a person needs. According to the Substance Abuse and Mental Health Services Administration’s 2024 National Survey on Drug Use and Health, approximately 48.4 million people aged 12 or older in the United States had a substance use disorder in 2024. However, many people who need treatment do not receive it. Connecting patients to the right level of care after discharge is where coordination between hospitals and addiction treatment providers becomes essential.

Northern Illinois Recovery Center, located in Crystal Lake, Illinois, is committed to helping people access the care they need after a medical crisis. The center works with healthcare professionals across the region to help bridge the gap between hospital discharge and ongoing addiction treatment.

When a hospital team identifies a patient who needs continued addiction treatment, Northern Illinois can assist with coordinating an appropriate next step, whether that is medical detox, residential treatment, or an outpatient program. Northern Illinois Recovery Center is accredited by The Joint Commission and licensed through the State Substance Use Treatment Agency (Outpatient facility license #A-2616-0001-A; Detox/Inpatient facility license #A-2616-0002-A), and is also recognized by NAATP and LegitScript.

Coordinated hospital partnerships reduce the gap between discharge and treatment by giving hospitals and addiction treatment providers a clear, shared process for connecting patients to the right level of care. When both sides communicate clearly, patients are less likely to fall through the gap between discharge and treatment. Coordinated referrals ensure continuity of care, match patients to an appropriate level of care to reduce delays, and give discharge teams a reliable point of contact.

Effective coordination depends on a few key elements working together:

  • Streamlined referrals: Connecting patients with treatment resources before they leave the hospital, rather than after
  • Continuity of care: Supporting a direct path from acute medical care into ongoing addiction treatment
  • Individualized planning: Identifying which level of care fits the person’s clinical needs, not a default option
  • Provider communication: Sharing relevant clinical information with appropriate patient authorization
  • Family and patient support: Helping individuals and their families understand what treatment options are available
  • Cross-setting coordination: Linking hospital services with detox, residential, outpatient, or behavioral health resources as appropriate

Every element above is centered on the individual’s needs and treatment goals, not on process for its own sake.

Our approach to client care encompasses mind, body,
and spirit as a unified whole.

What Is the Value of Healthcare and Addiction Treatment Partnerships?

Healthcare and addiction treatment partnerships support better outcomes by reducing the time between a medical crisis and the start of structured treatment. According to the National Institute on Drug Abuse, addiction is a chronic condition that responds to treatment, and early engagement following a health crisis can be a meaningful point of intervention.

When healthcare and treatment providers work together, patients benefit from:

  • A clearer path from hospital discharge to the appropriate level of care
  • Reduced risk of relapse or readmission during the transition period
  • Treatment planning that accounts for co-occurring medical or mental health conditions
  • Families who are informed and involved in next steps

Partnerships are not about volume or referral quotas. They are about making sure that when a person is ready to accept help, the right resources are available and accessible.

How Is the Transition From Hospital to Treatment Coordinated?

The transition from hospital to addiction treatment works best when both sides communicate early and clearly. A treatment coordinator reviews the individual’s clinical needs, identifies an appropriate level of care, and works with the hospital discharge team to coordinate timing and admissions, all with the patient’s consent.

The coordination process generally involves:

  • Reviewing the individual’s treatment needs and any relevant clinical history
  • Communicating directly with hospital discharge planners or social workers
  • Identifying whether detox, residential, or outpatient care is the appropriate starting point
  • Coordinating admissions to minimize the gap between discharge and treatment entry
  • Providing the referring team with information about available services
  • Maintaining communication with the referring provider as appropriate after admission

All transitions are coordinated according to the patient’s needs, clinical recommendations, and applicable consent requirements. The goal is a handoff that is clear, timely, and appropriate for the individual.

Who Collaborates in Addiction Recovery Across Settings?

Addiction recovery typically involves a range of professionals working across different points in a person’s care. Recovery rarely happens in a single setting or with a single provider. Northern Illinois Recovery Center’s role is to work alongside hospital-based teams, not to replace them.

Professionals who may be involved in coordinating addiction recovery include:

  • Physicians and nurses managing acute medical care
  • Hospital discharge planners coordinating next steps
  • Social workers and case managers identifying community resources
  • Behavioral health professionals addressing co-occurring mental health needs
  • Families and caregivers supporting the individual through the transition
  • Addiction treatment professionals providing structured clinical care after discharge

When these roles communicate and coordinate, the individual receives more consistent support across the full continuum of care.

How Do You Connect Patients With the Right Level of Care?

The right level of addiction treatment care is determined by a clinical assessment of the individual’s needs, not a standard pathway. According to the American Society of Addiction Medicine’s criteria, placement decisions should be based on an assessment across multiple dimensions, including withdrawal risk, medical conditions, and social supports.

Levels of care that may be appropriate depending on clinical need include:

  • Medical detox: For individuals who require medically supervised withdrawal management
  • Residential treatment: For those who need structured, 24-hour support during early recovery
  • Partial hospitalization (PHP): Intensive daily programming without overnight stays
  • Intensive outpatient (IOP) or outpatient treatment: Structured care that allows the individual to live at home
  • Continuing recovery support: Aftercare, sober living, and ongoing outpatient services
